COVID, Our Practice, and You
Rest assured we take the dangers of COVID very seriously. All patients and staff are required to wear a mask at all times. Patients are screened for symptoms prior to their appointment and will have their temperature taken upon arrival. Anyone who is not feeling well and potentially symptomatic will be rescheduled as soon as it is safe to do so.
Your Initial Appointment
Our goal is to schedule your appointment as soon as possible after receiving an abnormal diagnosis. We will do our best to see you within a week if we can.
We request new patients arrive at least 15 minutes ahead of your appointment to complete registration forms. You may also complete the forms below ahead of time and bring them with you or fax them to our office..
Please be sure to send any records to our office prior to your appointment. Forms and records can be faxed to 302-200-9131. Some images we may be able to obtain electronically, and others will have to be sent on a disc. Please be sure your disc arrives prior to your appointment.
